| Delivery Address:  Delivery address is the same as the billing address  I wish to send the order to a different address(see below) To prevent us and you from fraud we can only deliver to another address if you provide us with the following via a fax or emailed scan:
															 
																
 1. A utility bill with the same name & address as the registered billing address of the card2. A statement from the credit card
 3. A scan or faxed photocopy of the front and back of the card.
 4. A signed authorisation letter.
 
 or Instead of the above we will accept a faxed headed letter or email from your card issuing bank verifying who you are and that you authorised the delivery to another address. Once we have received an order with a different delivery address we will wait 5 days for the above documentation, after 5 days we will cancel the order and your card will not be charged. |
